Enrollment Application


We are now accepting applications for the 2009-2010 school year.

Step 1: Register

If you have not already done so, register online to save your spot in iQ Academy.

Step 2: Enroll

Once registered, fill out and print the Enrollment Application and submit to iQ Academy by fax, mail, or email.*

Required Documents

Upon receipt of your Enrollment Application, a request for release of the following records to iQ Academy Minnesota may be made:

  • Courses and Grades Completed
  • Special Education Records
  • State Test Results
  • MARSS #
  • Health/Immunization Records

Step 3: Select Courses

Once your Enrollment Application is processed, an iQ Academy school counselor will contact you to help you select your courses.
